Mail, fax, or hand deliver the completed Enrollment Packet to our office. You will also need to include the following items:  

  • Birth Certificate (original state issued)
  • Proof of Address (utility bill, lease agreement, mortgage receipt)
  • Immunization Records (dates of vaccinations signed by physician) Click HERE for requirements
  • Special Education Records (if applicable)
  • Withdrawal Form (including date of previous school exit)
  • Transcript
  • Custody or Guardian Papers (if applicable)

  • The HHSO staff will review your transcript and enrollment paperwork and contact you within 2 business days to schedule an enrollment appointment.
